A presentation and showing of the film Homeward Bound as part of a carers event to talk about system integration for end of life care. 

The workshop will have commissioners and providers from across Surrey Heartlands including Clinical Commissioning Groups, Acute and community hospitals, carers support organisations, primary care, local authority and voluntary sector organisations.
